Emergency / Aftercare form

It is required that EVERY family has an emergency/aftercare form on file in the school office. The office uses this information to contact family or designated people when a need arises for the student.  Please fill out the form and return it to the school office as soon as possible.  We have consolidated three other forms into this one for your convenience, we only require one from per family.
